By booking an appointment or sending a message through this website, you agree to the following terms.
Data controller
The controller of your personal data is Tomasz Kaputa, who runs the Dialegesthai online psychotherapy practice. Contact: [email protected].
What data, why, and on what legal basis
- Booking an appointment (name, email, phone number, time slot): to arrange and hold the session and to contact you about it. Legal basis: Art. 6(1)(b) GDPR (steps prior to entering into a contract, and performing it).
- Contact form (name, email, message): to reply to your message. Legal basis: your consent, Art. 6(1)(a) GDPR, and, if your message includes information about your health, Art. 9(2)(a) GDPR. You can withdraw your consent at any time; this does not affect the lawfulness of processing before withdrawal.
- Visit statistics: this site uses Cloudflare Web Analytics, which sets no cookies and does not track you across sites. Legal basis: the controller's legitimate interest, Art. 6(1)(f) GDPR.
Your data is not used for marketing or for automated decision-making, including profiling. Providing data is voluntary, but without it I cannot book a session or reply to your message.
Who processes data on my behalf
- Cal.com: appointment booking;
- Google (Google Calendar, Google Meet): appointment calendar and online sessions;
- Formspree: contact form delivery (Formspree Privacy Policy);
- Zoho: email (EU servers);
- Cloudflare: website hosting and visit statistics.
Cal.com, Google, Formspree and Cloudflare may process data in the United States. Such transfers rely on the European Commission's adequacy decision (EU-US Data Privacy Framework) or on standard contractual clauses approved by the Commission.
How long data is kept
Booking data is kept for the duration of our work together and afterwards for as long as the law requires or until any possible claims become time-barred. Contact form messages are kept until our correspondence ends or you withdraw your consent.
Your rights
You have the right to access, rectify and erase your data, to restrict its processing, to data portability, and to object to processing based on legitimate interest. You also have the right to lodge a complaint with the Polish supervisory authority, the President of the Personal Data Protection Office (UODO, ul. Stawki 2, 00-193 Warsaw, uodo.gov.pl).
